Event Completion Forms!

Event Completion forms should be fnished within two weeks of attending a workshop. They give you a chance to reflect on your experience and ensure you receive credit for your workshop attendance. This is an online tool that will take you approximately five to ten minutes to complete. Simply choose your certificate from the drop down menu below, select the specific workshop you attended, and complete the online survey. If you are unsure of which certificate is associated with your workshop, choose the Leadership Enrichment option and it will include forms for all workshops.
